Steroids and Cholesterol: Protecting Your Heart
Lower HDL and higher LDL are among the most consistent effects of anabolic steroids, and they add to heart risk over time. Here is what to measure and what helps.

Cholesterol travels in the blood inside particles called lipoproteins. Low-density lipoprotein (LDL) carries cholesterol into artery walls, where it can build up as plaque. High-density lipoprotein (HDL) helps carry cholesterol away. Anabolic steroids push both in the wrong direction, and over years that matters for your heart.
What steroids do to your lipids
- HDL falls. Androgens increase an enzyme in the liver (hepatic lipase) that breaks down HDL. Drops of 20 to 70 percent have been recorded, depending on the compound and dose.
- LDL often rises. Especially with oral compounds.
- Triglycerides vary. They may fall or rise depending on diet and compound.
Which compounds are worst
| Compound type | Typical effect on lipids |
|---|---|
| Oral 17-alpha-alkylated (stanozolol, oxandrolone, methandrostenolone, oxymetholone) | Fastest and largest HDL drops; LDL often up |
| Trenbolone | Large HDL drops reported |
| DHT derivatives (drostanolone, methenolone) | Moderate HDL drops |
| Testosterone at clinical doses | Small changes |
| Testosterone at high doses | Moderate HDL drop |
Part of testosterone’s gentler effect comes from its conversion to estrogen, which supports HDL. Using aromatase inhibitors to push estrogen very low can make lipids worse.
Why it matters for your heart
Imaging studies of long-term steroid users have found more coronary artery plaque than in similar non-users, along with thicker heart muscle and reduced pumping function in some. Heart attacks in young, otherwise fit steroid users are documented in case reports. Lipid changes are one part of this; raised blood pressure, thicker blood and direct effects on the heart muscle are others.
Canadian labs report cholesterol in mmol/L. Targets depend on your overall risk, so discuss your numbers with your doctor.
Testing
Get a lipid panel before starting, about six weeks in, and six to twelve weeks after stopping. Ask about apolipoprotein B (ApoB), which Canadian guidelines include as a useful marker of the number of harmful particles. Non-fasting tests are acceptable for most people.
What helps
- Cardio: 150 minutes a week of moderate cardio raises HDL modestly and improves blood pressure.
- Fibre: oats, barley, beans, lentils and psyllium lower LDL.
- Fats: swap saturated fats (fatty meats, butter, coconut oil) for unsaturated (olive and canola oil, nuts, fish).
- No smoking: smoking lowers HDL and multiplies heart risk.
- Limit alcohol: it raises triglycerides and blood pressure.
- Avoid oral compounds and keep doses modest: the biggest levers on steroid-related lipid changes.
Blood pressure and lipids together
High blood pressure and poor cholesterol multiply each other’s damage to arteries. Many steroid users have both at once, along with thicker blood. Treating them as one problem makes sense: measure blood pressure at home weekly, get lipids checked on the same schedule, and if either moves sharply, reduce or stop the compounds responsible rather than only adding supplements or medicines on top.
After stopping
HDL and LDL usually move back toward baseline over one to three months. Changes in heart structure take longer, and some may not fully reverse after long, heavy use. A follow-up lipid panel confirms recovery; if numbers stay abnormal, your doctor can assess whether treatment is needed.
Questions
How fast does HDL drop?
With oral steroids such as stanozolol, studies recorded large drops within weeks. Injectable testosterone at moderate doses has a smaller effect.
Does fish oil fix steroid cholesterol changes?
Omega-3s lower triglycerides but do little for HDL or LDL changes caused by steroids. They are not a fix.
Will my cholesterol go back to normal after stopping?
In most people lipids recover over one to three months, but long or heavy use may leave lasting changes in the heart and arteries.
Should I take a statin?
That is a decision for your doctor based on your overall risk. Statins lower LDL effectively but do not raise HDL much.
