Oral Steroids Explained: How Tablets Differ From Injections
Tablets feel like the easier choice: no needles, no syringes. Chemically, they are usually the harder one on your body.

Many people start with tablets because they want to avoid injections. It is an understandable instinct and, chemically, usually a mistake. This guide explains what makes an oral steroid work, why that same feature causes most of its problems, and how the common tablets compare.
Why most steroids cannot be swallowed
When you swallow a pill, it passes through the liver before reaching the rest of the body. The liver breaks down natural testosterone so efficiently that almost none of a swallowed dose would survive. Chemists solved this by adding a methyl or ethyl group at the 17-alpha position of the steroid, which blocks that breakdown. The result is called a 17-alpha-alkylated steroid, and it describes nearly every oral steroid you will see.
The price of surviving the liver
- Liver strain. The same structure that resists breakdown stresses liver cells and bile flow. Raised liver enzymes are common; cholestasis, peliosis hepatis and liver tumours are rare but documented with long use.
- Cholesterol. Orals act directly on how the liver handles fats, lowering HDL faster and further than injectables.
- Short half-lives. Most clear within hours, so they are taken more than once a day.
- Same suppression. Natural testosterone falls just as it does with injections.
The common orals at a glance
| Compound | Medical history | Notes |
|---|---|---|
| Oxandrolone (Anavar) | Weight regain after trauma or surgery | Least liver-toxic of the 17-aa orals; large HDL drop |
| Methandrostenolone (Dianabol) | Withdrawn | Converts to a potent estrogen; water retention |
| Stanozolol (Winstrol) | Hereditary angioedema | Severe HDL drop; joint aches |
| Oxymetholone (Anadrol) | Anemia | Blood pressure, headaches, liver strain |
| Chlorodehydromethyltestosterone (Turinabol) | East German sport programme | Never a mainstream medicine |
| Mesterolone (Proviron) | Male hypogonadism (some countries) | Not 17-aa; weakly anabolic |
Why “oral only” is not a gentle start
Using tablets alone still shuts down natural testosterone, so you end up with low testosterone plus the liver and lipid costs of the oral. Medical testosterone injections, by contrast, have decades of safety data at clinical amounts. That is why the protocol builder treats testosterone as the base and limits tablets to short runs.
The non-17-alpha exceptions
A few androgens are active by mouth without the 17-alpha modification. Mesterolone uses a methyl group at a different position. Testosterone undecanoate is absorbed through the lymphatic system when taken with fatty food. Methenolone acetate is a weak oral that was once marketed. These are gentler on the liver, but each is weaker or more awkward to use, which is why most oral steroids in circulation are 17-alpha-alkylated.
Who should avoid oral steroids
- Anyone with liver disease, raised liver enzymes, or who drinks most days
- People with low HDL, high LDL or a family history of early heart disease
- Anyone with high blood pressure or kidney disease
- People taking warfarin or diabetes medicines
- Women (most orals masculinise strongly), men trying to conceive, and anyone under 21
Stacking orals multiplies risk
Two oral steroids taken together act on the same androgen receptor, so the muscle-building effect does not add up neatly. The liver and cholesterol effects, however, do. If oral use is being considered at all, one oral for a short period, with bloodwork, is the more cautious approach.
Tablet quality
Many oral steroids today are made outside the pharmaceutical system, and studies of seized products have repeatedly found tablets with less, more or different drug than the label says. Look for brands that print strength, tablet count, batch number and expiry on the packaging, and be wary of unbranded tablets sold in bags.
Monitoring with orals
Liver panel (ALT, AST, GGT, bilirubin) and lipids before starting and two to four weeks in, plus blood pressure at home. Avoiding alcohol and high doses of acetaminophen reduces extra strain on the liver.
Legal status
All of these are Schedule IV anabolic steroids under the Controlled Drugs and Substances Act. Possession for personal use is not an offence under the Act; selling, importing and exporting without authorisation are.

Questions
Are oral steroids safer for beginners?
Usually the opposite. Avoiding needles does not avoid liver strain, cholesterol changes or suppression. A testosterone injection is generally the better-studied starting point.
Why are orals taken several times a day?
Their half-lives are short, often a few hours, so levels fall quickly between doses.
Which oral is easiest on the liver?
Mesterolone (Proviron) is not 17-alpha-alkylated and is much gentler. Among the 17-alpha-alkylated orals, oxandrolone is considered the least toxic, but none are risk-free.
Do I need to stop alcohol on orals?
Alcohol adds to liver strain and to some blood pressure effects. Most doctors would advise avoiding it entirely while taking a 17-alpha-alkylated oral.
