Anadrol vs Dianabol: How the Two Classic Orals Compare

Both are fast-acting oral steroids with long histories. They differ less than gym lore suggests, and their risks overlap almost completely.

Anadrol (oxymetholone) and Dianabol (methandrostenolone) are the two oral steroids most associated with rapid size. They are often compared as if one were gentle and the other harsh. The real differences are smaller, and the shared risks matter more.

Side by side

Anadrol (oxymetholone) Dianabol (methandrostenolone)
Origin DHT derivative Testosterone derivative
Medical use Anemia (still approved in the US) Withdrawn; once used for muscle wasting
Usual tablet 25 or 50 mg 10 or 20 mg
Half-life About 8 to 9 hours About 3 to 6 hours

How they feel different

Users describe Anadrol as heavier hitting in the first weeks, with more headaches, appetite loss and lethargy. Dianabol is described as producing a “pump” and a sense of well-being, with puffiness linked to its estrogen conversion. These are anecdotal patterns rather than trial findings; individual responses vary widely.

Where they are the same

  • Both are 17-alpha-alkylated, so both raise liver enzymes and carry rare but serious risks such as cholestasis.
  • Both lower HDL cholesterol quickly.
  • Both raise blood pressure, partly through water retention.
  • Both suppress natural testosterone within days.
  • Much of the weight gained with either is water and glycogen that leaves after stopping.

Why combining them is a bad idea

Taking two 17-alpha-alkylated orals at once does not double the muscle-building effect, because both act on the same receptor. It does add their liver and cholesterol effects together. If you are weighing either compound, that is the single most important point.

Medical background, briefly

Oxymetholone is still approved in the United States for anemias caused by poor red blood cell production, which is why its label is detailed about liver monitoring. Methandrostenolone was approved there in 1958 for conditions involving muscle loss and withdrawn decades later. Neither is marketed in Canada. Both are now produced mostly outside the pharmaceutical system, so tablet strength and quality vary between makers.

Strength per tablet versus strength per milligram

Methandrostenolone is more potent per milligram. Oxymetholone is weaker per milligram but comes in larger tablets, commonly 50 mg. Comparing “one tablet of each” is therefore comparing very different amounts of drug. The useful comparison is the effect and the side effects at the amounts people actually take, and there the profiles are similar.

Who should avoid both

  • Anyone with liver disease or raised liver enzymes, or who drinks heavily
  • People with high blood pressure, heart disease or kidney disease
  • Anyone with low HDL or a family history of early heart disease
  • Women, men trying to conceive, and anyone under 21

Interactions shared by both

Both strengthen the effect of warfarin and other blood thinners and can lower blood sugar in people on diabetes medicines. Alcohol, large amounts of acetaminophen and other liver-processed drugs add to the strain.

Myths and facts

Myth Fact
Anadrol is the “harsh” one and Dianabol the “gentle” one Both are 17-alpha-alkylated orals with similar liver, lipid and blood pressure effects.
Kickstarting with an oral is necessary Long esters reach steady levels in a few weeks on their own; an oral adds risk mainly for faster scale weight.
Water weight is harmless Fluid retention raises blood pressure, which is the main short-term cardiovascular risk.

What happens after stopping either one

Both clear the body within a day or two of the last tablet, so the water and glycogen they added begin to leave within the first week. Scale weight usually drops by a few kilograms, which is why gains from orals alone often look disappointing a month later. Liver enzymes typically settle over several weeks, and HDL recovers over one to three months. Natural testosterone production, suppressed within days of starting, also needs time to restart, and a blood test six to eight weeks after stopping is the only way to know it has.

Monitoring

For either one: liver panel, lipids, blood pressure, complete blood count, estradiol and total testosterone, ideally before starting and two to four weeks in.

Questions

Which is 'safer'?

Neither. Liver, blood pressure and cholesterol effects are similar in kind. Shorter use, lower amounts and monitoring matter more than the choice between them.

Which causes more water retention?

Both do. Dianabol's comes partly through estrogen; Anadrol's through mechanisms that are less clear. Users report heavy bloating with either.

Can I take them with a liver supplement and be fine?

No supplement has been shown to prevent liver injury from these drugs. Bloodwork is the only way to know what is happening.

Why do these orals crash HDL so quickly?

Oral 17-alpha-alkylated steroids act directly on the liver's lipid handling, which lowers HDL faster than injectables.
